I acknowledge with my signature that I am accepting full responsibility for this firearm and its safe use. I will learn the proper way to take the gun apart and put it back together correctly, maintain the shotgun by cleaning it regularly, wiping it down and using a pull-through after every use. I will handle it properly, being certain not to drop it or damage it in any way, storing it properly, unloaded and secured. I agree to return the shotgun to Alaska SCTP or my head coach at the end of the season or when I am no longer shooting with the team in as good a condition as when I borrowed it, clean and functioning properly, with no damage to the firearm. I understand that any repairs or cleaning fees will be my responsibility or the responsibility of my parents or guardians, and must be paid upon presentation of the bill. If the gun is lost, stolen or damaged beyond repair, the full stated value of the firearm must be paid immediately. If lost, stolen, or not returned by the athlete, law enforcement will be notified.

A fee of $100.00 is required for borrowing a shotgun. The $100.00 will be refunded provided that the shotgun is returned in good condition, thoroughly cleaned, and functioning properly. If repairs or cleaning are required, the athlete and their parents will be responsible for the cost of cleaning and repairs.
